Tracey Ross, BSN, LNHA

Tracey has been working in the field of Developmental Disabilities for over 22 years and has been the Administrator at Foundations since 2007. Her career at Foundations began as the Director of Nursing from 1995 until 2007. Prior to her service at Foundations, Tracey worked as a Registered Nurse at Reid Hospital gaining strong nursing skills while working in the Emergency Department. Tracey has made a commitment throughout her career to assuring the health and safety of the individuals she serves and has become an advocate for individuals who cannot speak for themselves. Tracey earned her Associates of Science in Nursing in 1991 from Indiana University East, she returned to college in 2003 and graduated with her Bachelor’s of Science in Nursing in 2004. In 2005, Tracey completed her Administrator in Training.

Foundations is an ICF/IID that specializes in the care of children and adults with mild to profound developmental disabilities and individuals who are behaviorally challenging. The facility is licensed by the Department of Development Disabilities and receives oversight by the Ohio Department of Health. Foundations offers 24/7 Licensed Nursing care, Physical Therapy, Occupational Therapy, Speech Therapy, a licensed Dietitian, Psychologist, and Case Management Services.

Each individual has a case manager (QIDP) and a team of professionals that meet regularly to ensure that each resident’s needs are met. All care that is provided is individualized. Foundations schedules and provides transportation to all medical appointments. The facility communicates and coordinates appointment information with the individual’s guardian.

In addition to Health related services, Foundations offers programs that assist individuals in maintaining skills and developing new skills. Individuals that reside at Foundations are actively involved in their community and participate in a number of activities throughout the year. Children are enrolled in the National Trail School District. Our residents look forward to Spring as they have been long time Red’s Season Ticket Holders!

In addition to ICF/IID Services, we are a certified provider of IO, Level 1, and Non-Medical Transportation in Preble County. This allows families a choice of settings with a range of service options.
