Jason grew up around health care, as his family owned a small nursing home in Preble County, Ohio.  He attended Eaton High School and is a 1996 graduate of Bowling Green State University, where he received a Bachelors degree in Music Education.  After teaching for a few years in Fremont, he returned to the healthcare field working for Woodlane Residential Services in Wood County, Ohio.  He later accepted a QIDP position at Foundations.  He also spent time as the Activities Director and Community Coordinator.  In the fall of 2022, he was promoted to Administrator.  Jason has 23 years of expereince in the DD field.  

Jason spent part of his school aged years in Tucson, Arizona where became involved in the school music program.  He continues to play trumpet with several locat community based groups.  He spent many years volunteering for locatl animal rescue organizations.

Foundations is an ICF/IID that specializes in the care of children and adults with mild to profound developmental disabilities and individuals who are behaviorally challenging. The facility is licensed by the Department of Development Disabilities and receives oversight by the Ohio Department of Health. Foundations offers 24/7 Licensed Nursing care, Physical Therapy, Occupational Therapy, Speech Therapy, a licensed Dietitian, Psychologist, and Case Management Services.

Each individual has a case manager (QIDP) and a team of professionals that meet regularly to ensure that each resident’s needs are met. All care that is provided is individualized. Foundations schedules and provides transportation to all medical appointments. The facility communicates and coordinates appointment information with the individual’s guardian.

In addition to Health related services, Foundations offers programs that assist individuals in maintaining skills and developing new skills. Individuals that reside at Foundations are actively involved in their community and participate in a number of activities throughout the year. Children are enrolled in the National Trail School District. Our residents look forward to Spring as they have been long time Red’s Season Ticket Holders!

In addition to ICF/IID Services, we are a certified provider of IO, Level 1, and Non-Medical Transportation in Preble County. This allows families a choice of settings with a range of service options.
